The Hospital Sírio-Libanês provides a Pre Admission Service. Its object is to make the hospitalization process both easier and quicker for the patient, as he or she can give all his or her data over the telephone in advance.
The patient must give the number of his or her passport or identity card, full name, address and telephone number amongst other information, as well as bringing authorization from the health plan for the procedure to which he or she will be subjected. On arrival at the Hospital, this data is quickly checked and the patient is transferred to his or her room.

Documents necessary:
Private patient
- Passport or, for patients coming from countries where a passport is not required to enter the country, bring a photo-identity document.
- For patients under 18 years old, the passport or identity document of the person responsible for the patient.
- Proof of address in the country of origin and of the place where the patient will stay in Brazil during the treatment to which he or she will be subjected.
- Admission request issued by the doctor, dated, stamped and signed.
Patients with Healthcare Insurance Plan
- Passport or, for patients coming from countries where a passport is not required to enter the country, bring a photo-identity document.
- For patients under 18 years old, the passport or identity document of the person responsible for the patient.
- Healthcare Insurance Plan or Health Insurance Identity Document.
- Proof of address in the country of origin and of the place where the patient will stay in Brazil during the treatment to which he or she will be subjected.
- Admission request issued by the doctor, dated, stamped and signed
- Healthcare Insurance Plan or Health Insurance authorization for the procedure to be carried out.
Recommendations
- Arrive earlier that the time booked so that the team can welcome you and treat you calmly.
- Don't forget to bring your laboratory and imaging tests, and any others related to your treatment.
